You contribute 12% of basic + DA. Your employer also contributes 12%, but up to ₹1,250 of it goes to the pension scheme (EPS) instead of your EPF.
How is EPF interest calculated?
On the balance each month, at the yearly rate ÷ 12, added to your account at the end of the year.

Your contribution counts for 80C under the old regime. Interest and withdrawals are tax-free after 5 years of service, except interest on contributions above ₹2.5 lakh a year.
What is VPF?
Voluntary PF: extra money you choose to put in, earning the same rate. It counts towards the ₹2.5 lakh limit for tax-free interest.
